3D dentistry according to the VieSID concept
Digital dentistry revolves around the true hinge axis

Abstract:

The surge of digital technics within the field of Dentistry has significantly affected the daily work of dental professionals. In prosthodontics, as in orthodontics and dental technologies, many processes are carried out virtually, from intraoral digital model creation to the morphing of splints, dentures or orthodontic appliances.
Despite the appeal of the digital techniques, both the dentists and the dental technicians shall aim to individualized treatment of the patient, based on a careful diagnosis, accurate treatment planning, still optimizing their workflows for efficient and cost-effective work.
With this presentation, I aim to discuss advantages and pitfalls of digital dentistry, and eventually, show innovative digital solutions and applications enabling implementation of occlusion medicine protocols in the virtual environment allowing to retain quality and accuracy. During such discussion, the importance of accurate identification of the patient´s hinge axis will become clear.

Togni Daniele

Lector — Togni Daniele graduated in 1988 at the University of Milan and some years later I joined the courses with prof. Cardonnet and prof. Rozencweiz.
Since 1994 there was a close cooperation with a surgeon to treat many of my patients with complex prosthetic rehabilitations on implants and this was the occasion for remarkable experience in the field of the latest digital radiology (CBCT) particularly for the diagnostics of the TMJ.
In 2005 I joined the courses at the Danube University of Krems (Austria) with Prof. Rudolf Slavicek and in 2011 I’ve got the title of Master of Science with a thesis about “Virtual articulator model in CAD software environment”.
In 2011 prof. R. Slavicek invited me to the international study group “Clubbing for science”: a quite interesting “multicultural environment» for scientific discussion where we worked together for many years.
In 2014, after some years of collaboration with Monica Casadei, I joined Prof. S Sato’s orthodontic course that was successfully completed in 2017.
From 2017 I was invited to be part of the VieSID scientific research group in Vienna: the permanent dialogue with the other members of the VieSID school is for me a fundamental educational experience.
Among the staff of VieSID, the Vienna school of Interdisciplinary Dentistry, I was one of the main responsible for the organization of the teaching material and since 2023 I am part of VieSID board with the responsibility of Didactic coordinator for the courses.
The holistic view of the patient’s problems is the only one I believe in; I want to practice a medicine based on solid scientific foundations and I am convinced that any dentistry without good gnathological knowledge is inadequate.
Since 1988 I successfully run my private dental practice as a self-employed in Lecco.
I’m happily married with Paola, and we have three children: Maddalena, Elisabetta and Martino.

«Individualized functional dentistry based on VieSID concept»

Abstract:
The clinical application of Evidence based medicine (EBM) has revealed treatment failures and flaws partly due to a distorted interpretation of its essence. In many fields of medicine, an approach accounting for the individuality of the patients is proving to be much more successful by considering their genetic characteristics and their individual response to environmental factors and drugs. This patient-tailored health-care model, referred to as «individualized medicine» or «precision medicine», has proven effective in treating cancer, cardiovascular diseases and other complex diseases. In this presentation, I will illustrate the main conceptual differences between individualized and evidence-based medicine, and will discuss the effects of the most common misinterpretations of these fundamental principles. I will also discuss the meaning of individualized medicine in dentistry by addressing the following questions: Are the concepts of individualized medicine applicable in dentistry? Are there areas of dentistry where the application of «individualized dentistry» could guarantee us greater clinical success than the EMB approach? How VieSID concepts support us in applying the “individualized functional dentistry” to be more successful in therapy?

The clinical and scientific evidence supporting mandibular repositioning
Abstract:
The clinical efficacy of mandibular advancement for TMJ orthopaedic problems, including disc derangement, joint degeneration, arthralgia and skeletal discrepancies has often been questioned, and some scholars have strongly argued against the application of irreversible oral treatments to solve craniomandibular pain and dysfunction. To address this long-standing debate, in this talk I will provide an overview of selected literature elucidating the matter. Different types of articles including experimental animal studies as well as clinical research and guidelines will be considered.
Experimental work conducted from the 1980s demonstrated the physiological response to mandibular advancement of both hard and soft TMJ tissues. Clinically, variable percentages of success in recapturing luxated discs and remodelling degenerating joints were reported. Notably, the literature consistently reported reduction of joint and muscle pain after TMJ orthopaedic treatment by mandibular repositioning. With this contribution, a synopsis of the most relevant literature supporting the validity of mandibular repositioning will be provided.

Chmielewska Johanna

Lector — Joanna Trybuła Chmielewska — In the dental technology, the greatest opportunity to express myself in artistic expression gave me the ceramics. To this day I am fascinated by the possibilities I give, layers, transparens, translucations, opalescences, light plays, texture, surface. A small, living work, a sculpture that becomes someone’s smile. Motivaiting is the discovery of new techniques, and the possibilities of new materials and technologies, DSD, cad / cam is my toy in dayly work .Work directly with the patient and his flash in the eye when he gets a new smile is the best feeling. After starting my education in Viesid with prof.
R. Slavicek, my passion became occlusion. The desire to thoroughly understand the processes taking
place in the masticatory system constantly pushes me to develop in this direction. The acquired skills
and new technologies enable the performance of more and more precise, individualized prosthetic
work, which is a safe rehabilitation of the patient.
Being aware that using the right tools, I can give the patient not only aesthetic but fully functional
work, A small, living work, a sculpture that becomes someone’s smile. Its gives me great satisfaction
and a motor for exploring the arcuate knowledge of occlusion.

Assessment of the function of the Craniomandibular system by clinical instrumental analysis

Abstract:

The Vienna School of Interdisciplinary Dentistry – VieSID concerns itself with individualized functional dentistry, and thus relies on a complete and accurate diagnostic protocol for treatment planning. Different diagnostic tools for the assessment of the stomatognathic system are applied to evaluate the static and dynamic determinants of occlusion in relation to the skeletal and joint structures to ensure harmonious function and long-term stability of the masticatory system.

Clinical instrumental analyses, obtained using tools directly applied to the patient, play an important role in the rehabilitation of the stomatognathic system by providing morphological and kinematic information. The clinical instruments used in the VieSID diagnostic protocol such as the occlusogram and the brux checker are intended for a direct evaluation of the occlusal functions, including bruxism. Condylography is used to evaluate the mandibular movements to identify possible functional limitations in relation to morphological alterations of the TMJ.

In this presentation, the technical aspects of the most relevant types of clinical instrumental analyses will be illustrated. Their clinical relevance will be discussed as well, based on cases diagnosed according to the VieSID protocol showing how oral functions like chewing, speech, bruxism, swallowing, and postural stabilization can be improved by correcting the occlusion, restoring orthopaedic balance in the temporomandibular joints and face, and eventually improving the patient’s facial aesthetics.

Presentation topic:

VieSID: Essential Concepts to Elevate your Game
Abstract:
VieSID provides postgraduate education in individualized functional dentistry based on the concepts taught to us by our mentor, Professor Dr. Rudolf Slavicek (1928 — 2022), an Austrian medical doctor, former director of the University Dental Clinic Vienna, Medical University of Vienna.
Professor Slavicek recognized that the health of the masticatory organ was an important factor in overall systemic well-being. Thus, the importance of proper function and the prevention and reduction of dysfunction are cornerstones of Vie-SID teachings. Accordingly, Slavicek established a comprehensive diagnostic protocol for the assessment of the clinical, functional and morphological aspects of the masticatory systems, which is essential before undertaking any dental procedure.
This fast-paced lecture will provide an overview of the concepts taught by VieSID. It will begin with an illustration of the clinical challenges we face in dentistry followed by a discussion of what sets VieSID apart in diagnosis and treatment planning. This lecture will serve as an introduction for those who are new to VieSID concepts and as a reminder of the core principles of occlusion medicine for those who are more experienced.
